Qualifications

• Valid Physical Therapy license in Minnesota (or state regulations requiring licensure)
• Current BLS certification (AHA/ARC) required

Responsibilities

• Evaluate patients’ condition using tests, history and exams to identify needs
• Develop personalized rehabilitation programs based on assessment findings and goals
• Implement treatment plans to improve mobility and reduce pain
• Document evaluations, interventions and outcomes accurately
• Collaborate with physicians and nurses to ensure coordinated care
• Educate patients and caregivers on home exercises and safety
• Maintain infection control and safety standards
• Participate in quality assurance and professional development
• Assist in discharge planning to support recovery in home settings
