Job summary

HealthForce Healthcare is seeking a Physical Therapist for a travel assignment in Sonora, CA. The role spans 13 weeks with a weekly pay range and EPIC charting required. Start on 10/12/2026, 36 guaranteed hours per week, day shifts from 8:00 AM to 4:30 PM, at a Short Term Acute Care Hospital.

The position demands a DPT or foreign equivalent, California licensure, CPR/BLS, a valid driver’s license, and at least two years of recent PT experience. EPIC documentation is required.

Qualifications

• DPT required; license in applicable states or foreign equivalent.
• Current CPR/BLS certification.
• Physical Therapy licensure in practice states.
• Valid Driver’s License; must be 21+.
• Minimum of 2 years of recent PT experience.
• EPIC experience for documentation and charting.

Responsibilities

• Plan and conduct rehab programs to restore function aligned with patient goals.
• Co-lead in interdisciplinary care planning with the team.
• Evaluate and create individualized treatment plans; select modalities.
• Provide interventions using approved protocols and communicate discharge plans.
