ProHealth Partners has excellent full-time opportunities in the Toledo area, requiring travel within a 40-mile radius to patients’ homes or facilities. Our team provides comprehensive support to focus on patient care.

Responsibilities include:

1. Supervising assistant therapy staff
2. Communicating with office staff for scheduling, paperwork, and payroll
3. Maintaining open communication with patients and home health agencies
4. Managing patient care in collaboration with Physical Therapist Assistants
5. Completing evaluations, care plans, reassessments, and discharges
6. Participating in meetings and case conferences

Requirements:

• Current Physical Therapist licensure
• Bachelor’s degree in Physical Therapy
• Ability to travel locally
• Strong communication skills

Home healthcare experience is preferred. Salary and per-visit compensation are negotiable.
