Job Responsibilities

• Supervise assistant therapy staff
• Communicate daily with office staff for patient intake, paperwork, and payroll
• Maintain open communication with patients and home health agencies
• Coordinate with Physical Therapist Assistants for patient care
• Complete evaluations, care plans, reassessments, and discharge paperwork
• Attend meetings and case conferences

Job Requirements

• Current Physical Therapist licensure
• Bachelor’s degree in Physical Therapy
• Ability to travel within a 40-mile radius of Toledo, OH
• Strong communication skills
• Home healthcare experience preferred

This position requires daily travel to patients' homes or facilities.

Salary is negotiable.
