Job summary

Apex Home Health, Hospice & Home Care is seeking a dedicated physical therapist to provide services across settings, evaluating, diagnosing, and treating patients with physical impairments while developing personalized treatment plans and collaborating with care teams.

You will use manual therapy and specialized equipment, document interventions, educate patients and caregivers, and participate in discharge planning, with a focus on safe practice and regulatory compliance.

Qualifications

• Valid physical therapy license and BLS certification.
• Strong clinical and communication skills; ability to document effectively.
• Ability to work under pressure and adapt to irregular schedules; PC proficiency.

Role Summary:

This position involves providing physical therapy services in various healthcare settings to evaluate, diagnose, and treat patients with physical impairments. The therapist develops personalized treatment plans, utilizes manual therapy and specialized equipment, and collaborates with healthcare teams to ensure optimal patient outcomes.

Responsibilities:

Conduct patient assessments, create and implement evidence-based rehabilitation programs, operate therapy equipment safely, monitor progress, and document interventions. Educate patients and caregivers on self-management, promote injury prevention, and participate in discharge planning. Maintain a safe environment and adhere to regulatory standards.

Qualifications & Skills:

Valid physical therapy license, current BLS certification, strong clinical and communication skills, critical thinking, and proficiency in documentation and PC use. Must have physical abilities to assist patients and handle equipment, excellent interpersonal skills, and the capacity to work under pressure and adapt to irregular schedules.
