Job summary

CONSUMER DIRECTED SERVICES INC. seeks a Physical Therapist (PT) to assess referred patients, develop individualized treatment plans, and provide skilled therapy under physician orders.

Responsibilities include patient evaluations, adjusting treatment programs, documenting progress, educating patients and families, and collaborating with healthcare teams to meet therapy goals.

Qualifications

• Doctorate or Bachelor’s degree in Physical Therapy required.
• Must pass NPTE.
• At least 1 year of experience.
• State licensure or in process.
• Current CPR certification.

Job description

Role: The Physical Therapist (PT) is responsible for assessing referred patients requiring rehabilitation, developing individualized treatment plans, and providing skilled therapy services in accordance with physician orders.

• Responsibilities: Conduct patient evaluations, establish and adjust treatment programs, document patient progress, educate patients and families, and collaborate with healthcare teams to meet therapy goals.
• Qualifications: Must hold a Doctorate or Bachelor's degree in Physical Therapy, pass the NPTE, have at least one year of experience, hold or be in the process of obtaining state licensure, and possess current CPR certification. Additional requirements may be specified by the client facility.
• Skills: Strong assessment and treatment planning abilities, excellent communication, teamwork, and documentation skills.
