Job Summary

Physical Therapist Outpatient in Falmouth, ME. The Physical Therapist evaluates assigned patients and develops, implements, and modifies individualized treatment plans based on patient needs, goals, and physician direction. The role provides outpatient therapy services, supports functional recovery and return to community activities, completes timely documentation, and contributes to patient education and professional development.

Job Details

• Schedule: Day shift, 8-hour shifts, with a minimum of 40 hours per week.
• Required Qualifications:
  • Degree in Physical Therapy from an accredited school
  • At least 1 year of outpatient clinical experience
  • Cardiopulmonary Resuscitation (CPR) certification
  • Active and applicable license in the state where services will be performed
• Preferred Qualifications:
  • Prior successful assignment experience is highly desired
  • Prior successful assignment with the organization is preferred
• Additional Information:
  • Complete comprehensive patient evaluations and develop treatment plans focused on progression toward discharge.
  • Provide discipline-specific therapy services in accordance with physician orders, patient needs, and applicable authorization requirements.
  • Complete accurate and timely medical record documentation, goal writing, functional quality assessments, medical necessity documentation, and charge entry.
  • Educate patients, families, students, coworkers, and other appropriate stakeholders.
  • Participate in student programs when assigned, including supervision, learning objectives, and evaluations.
  • Maintain professional licensure in good standing and complete required continuing education.
  • Follow applicable state practice standards, regulatory requirements, professional codes of conduct, safety procedures, and quality improvement initiatives.
