Job Summary

Physical Therapist, Outpatient Outpatient setting. The Physical Therapist evaluates patients, develops individualized treatment plans, and provides therapy services designed to support functional recovery and return to community activities. The role includes patient and family education, clinical documentation, coordination of care under valid physician orders, and participation in professional development activities.

Job Details

• Schedule: Day shift, 8 hours per shift, with a minimum of 40 hours per week. Assignment dates are 11/23/2026 through 02/22/2027.
• Required Qualifications:
  • Degree in Physical Therapy from an accredited school
  • At least 1 year of outpatient clinical experience
  • Cardiopulmonary Resuscitation (CPR) certification
• Additional Information:
  • Complete comprehensive patient evaluations and develop treatment plans and goals supporting progression toward discharge.
  • Document evaluations, treatment plans, goals, functional quality indicators, medical necessity, and charges accurately and timely using approved formats.
  • Provide care under signed, valid physician orders and within applicable clinical authorization requirements.
  • Delegate and direct discipline-specific therapy services for assigned patients.
  • Participate in student education and provide appropriate supervision when assigned.
  • Maintain professional licensure in good standing and complete required continuing education.
  • Follow applicable state practice acts, regulatory requirements, professional standards, safety procedures, and confidentiality expectations.
  • Promote patient and employee safety through safe patient handling, materials handling, infectious disease awareness, fall prevention, and hazard reporting.
