Job summary

Healthforce is seeking an Occupational Therapist for a travel assignment in Clearlake, CA. Start 10/05/2026 for 13 weeks, with a guaranteed 40 hours per week and day shift (Mon-Fri, 8:00 AM–4:30 PM) in a Critical Access Hospital.

The role requires CA OT licensure, BLS, and 2+ years of OT experience, with outpatient focus and some inpatient coverage. Remote support provided by Healthforce throughout the assignment.

Qualifications

• Active California license to practice as an Occupational Therapist.
• BLS certification is required.
• At least two years of recent OT experience.

Responsibilities

• Conduct OT evaluations and re-evaluations for outpatient, with occasional inpatient coverage.
• Develop and implement individualized treatment plans for ADLs, mobility, cognition, and fine motor skills.
• Provide direct OT interventions, monitor progress, and adjust plans as needed.
• Document progress and communicate with the care team; collaborate with physicians, nursing, and rehab staff.
• Educate patients and families on home exercise programs, safety, and discharge planning.
• Ensure compliance with regulatory standards and facility policies.
