Job summary

Healthforce Healthcare LLC in Clearlake, CA seeks an Occupational Therapist – General for a travel assignment. Start date 10/05/2026 for 13 weeks, daytime outpatient focus with some inpatient coverage in a Critical Access Hospital setting.

You will conduct OT evaluations, develop individualized treatment plans addressing ADLs, mobility, cognition, and fine motor skills, document progress, educate patients and families, and collaborate with the care team.

Qualifications

• Active California license to practice as an Occupational Therapist.
• BLS certification.
• At least two years of recent OT experience.
• Ability to work daytime schedule in outpatient settings with some inpatient coverage.
• Must be able to complete credentialing and participate in required orientation.

Responsibilities

• Conduct OT evaluations and re-evaluations for patients in outpatient, with occasional inpatient coverage.
• Develop and implement individualized treatment plans addressing ADLs, mobility, cognition, and fine motor skills.
• Provide direct OT interventions, monitor progress, and adjust plans as needed.
• Document patient progress and communicate with the care team; collaborate with physicians, nursing, and other rehabilitation professionals.
• Educate patients and families on home exercise programs, safety, and discharge planning.
• Ensure compliance with regulatory standards and facility policies.
