Job summary

American Traveler is seeking a travel Home Health Occupational Therapist for a 13-week assignment in Grand Junction, Colorado. The role covers home health OT services across multiple sites with a 40-hour workweek and day shift.

Require 2–3 years of OT experience, NBCOT certification, and Colorado OT licensure or ability to obtain. This position serves older adults in a PACE program and involves travel between sites.

Qualifications

• Bachelor’s, Master’s, or Doctorate in Occupational Therapy from an accredited program.
• Current NBCOT registration.
• Current Colorado Occupational Therapist license required, or ability to obtain before employment.
• 2–3 years of Occupational Therapy experience.
• Experience working with older adults or geriatric populations.

Responsibilities

• Evaluate, plan, and implement individualized OT treatment programs for daily living and work skills.
• Conduct home-based safety assessments and provide skilled interventions in day centers and homes.
• Coordinate DME and other supportive services to promote participant independence.
• Collaborate with physicians, nurses, therapists, social workers, and other healthcare professionals as part of an interdisciplinary team.
• Support participants' ability to safely remain in their homes and communities.
