American Traveler is hiring an Occupational Therapist with 2-3 years of experience for a PACE program supporting older adults through home, day center, and community-based care. The role includes evaluating, planning, and implementing individualized treatment plans to help patients develop, recover, or maintain daily living and work skills.

The position requires current NBCOT registration, Colorado OT license (or ability to obtain), and travel between sites.

Qualifications

• OT degree from an accredited program (Bachelor, Master, or Doctorate).
• NBCOT registration required.
• Colorado OT license required or ability to obtain before employment.

Responsabilités

• Evaluate, plan, and implement individualized treatment programs to help patients develop, recover, or maintain daily living and work skills.
• Conduct home-based safety assessments and provide skilled therapeutic interventions in the day center.
• Coordinate DME and other supportive services to promote independence.
• Collaborate with physicians, nurses, therapists, social workers and other healthcare professionals as part of an interdisciplinary care team.
• Support participants' ability to safely remain in their homes and communities.
