Responsibilities

• Provide evidence-based OT interventions to address upper-extremity function, fine motor skills, balance, and coordination
• Monitor progress, adjust treatment plans as needed, and document outcomes in the resident's chart
• Collaborate with an interdisciplinary team (nursing, therapy, social work, and rehabilitation aides) to support discharge planning and care goals
• Ensure safety, infection control, and adherence to facility policies and regulatory requirements during therapy sessions
• Educate residents and families on activities and strategies to maintain progress and prevent decline
