Job summary

Optum Home & Community Care, part of UnitedHealth Group, is hiring a Nurse Practitioner for HouseCalls statewide travel across Pennsylvania. This field-based role delivers in-home assessments for Medicare Advantage and other plans, with flexible scheduling and no on-call requirements.

Travel is about 85% across PA; base salary plus incentives, $40,000 student loan repayment or $25,000 sign-on bonus. Comprehensive benefits include travel expenses covered while practicing in a supportive team

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted state licensure without disciplinary actions.
• Ability to obtain licensure/start date requirements by start date.
• Active and unrestricted driver’s license with access to reliable transportation to complete home visits
• Ability to navigate varied home environments; ability to sit/stand/kneel as needed to perform assessments
• If your state allows independent practice for NPs/PAs, obtain approval from licensing board and pursue eligibility within specified timelines

Responsibilities

• Conduct annual in-home health assessments for Medicare Advantage and other plans.
• Review past medical history, medications, vitals, and perform exams.
• Educate patients on disease processes, medications and adherence.
• Coordinate with PCPs to address care gaps and plan management.
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on disease processes, medications, and compliance
• Address social determinants of health and provide referrals as needed
