Travel Nurse Practitioner

Position: Travel Nurse Practitioner

Monogram Health is seeking a skilled Nurse Practitioner eager to make a difference in patients' lives. The Travel Nurse Practitioner plays a critical role in providing access to comprehensive care for underserved patients within our Monogram Health network. The role involves helping patients improve their quality of life at home and slow the progression of kidney disease and other chronic illnesses, aiming for positive health outcomes.

Your Impact:

As a Travel Nurse Practitioner, you will deliver personalized and compassionate care primarily to patients with CKD through ESRD. You will serve as a nationwide float provider, reaching patients in areas where local clinicians are unavailable or in rapidly growing regions. Responsibilities include patient health assessments, developing health management strategies, and promoting healthy habits. You will also maintain accurate records, schedule and follow up with patients across multiple markets.

Roles and Responsibilities:
  • Travel nationwide for in-home assessments, wellness exams, screenings, and virtual visits
  • Travel up to 75% of the month during work weeks
  • Counsel and educate patients and families about health benefits and programs
  • Document all patient interactions thoroughly
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Complete documentation promptly
  • Maintain quality care standards
  • Work remotely based in your state, attending periodic training outside your state
  • Provide evidence-based care to reduce hospitalizations and improve quality of life
  • Prescribe medications, order tests, collaborate with physicians
  • Perform care gap closure and patient education
  • Participate in care team meetings
  • Assess patient health, implement care plans, and evaluate outcomes
  • Perform other duties as assigned
Position Requirements:
  • Active, unrestricted RN and Nurse Practitioner licenses
  • Board certification (ANCC/AANP)
  • Current DEA certificate or eligibility
  • Ability to travel extensively by air and car
  • Work independently without supervision
  • Valid driver’s license and insurance
  • Proficiency with medical instruments and electronic records
  • Effective communication skills
  • Knowledge of federal and state regulations (HIPAA, OSHA)
  • Minimum 2 years of direct patient care experience
