Job summary

This role emphasizes independence, proper documentation, and travel between sites to support transitions from hospital to home. Required are an active Colorado NP license and board certification, with CMS-compliant practices and EMR proficiency.

Qualifications

• Active and unrestricted Colorado NP license.
• Board-certified NP (AANP/ANCC) with AGACNP/ACNP/FNP/GNP focus.
• Ability to move a 30-pound bag and navigate stairs in varied dwellings.
• Reliable transportation to travel between client sites within area.
• Proficient documentation in electronic medical records (EMR).

Responsibilities

• Perform comprehensive assessments and document findings per CMS regulations.
• Ensure diagnoses are ICD-10 coded and properly documented.
• Conduct initial medication reviews.
• Travel between care sites is mandatory.
• Coordinate care and communicate with patient and team members.
