Job summary

UnitedHealth Group is seeking a Nurse Practitioner for in-home care in Colorado. This role involves daily travel to patient homes within the assigned area, working with an interdisciplinary team to coordinate preventive care and navigate the health system.

The APC will perform in-home assessments, manage acute and sub-acute conditions, and engage in advanced illness planning and caregiver education. A physician-led team will support your professional growth, with a strong emphasis on

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Colorado licensure or ability to obtain by start date.
• Unrestricted driver’s license and reliable transportation.
• DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to gain a collaborative practice agreement if required.

Responsibilities

• Conduct comprehensive in-home assessments including med history, vitals and exam.
• Manage members through acute events and chronic condition exacerbations.
• Provide short-term sub-acute rehab management and documentation.
• Document accurately including ICD-10 codes; collaborate with care team.
• Engage in advanced illness conversations and patient/c caregiver education.

Education

ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

Colorado licensure or ability to obtain by start date

Driver's license (unrestricted)

DEA licensure/prescriptive authority or ability to obtain post-hire

Collaborative practice agreement if applicable
