Job summary

Sweetwater Care in California seeks an experienced Licensed Vocational Nurse (LVN) to provide nursing care within scope of practice, following physician orders, center policies and state guidelines. You will document changes, participate in rounds, and support quality life initiatives while adhering to reporting procedures and company policies.

Familiar with long-term care, charting, and incident reporting; BLS/CPR certification required.

Qualifications

• Experience in a long-term care setting.
• Familiarity with PointClickCare charting.
• Active LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to obtain.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedures for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions (nutrition, infections, skin issues, weight loss, etc.).
• Uses Quality Life Manual to identify and monitor patients in appropriate programs.
• Documents and notifies departments of room changes, LOA, admissions, or discharges.
• Makes rounds with physicians when necessary.
