Job summary

Sweetwater Care in California seeks a licensed LVN to provide nursing care within scope of practice, support the Quality Improvement Program, and coordinate patient care in a long-term care facility.

You will administer medications and treatments, document orders, participate in rounds with physicians when needed, and educate families and patients, while ensuring compliance with center policies and state guidelines.

Qualifications

• Must hold an LVN license in California.
• One year of nursing management experience preferred.
• Experience in SNF/ LTC settings preferred.
• Long-term care experience highly regarded.
• Proficiency with PointClickCare charting.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring.
• Notifies other Departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders (if allowed by Nurse Practice Act).
• Makes rounds with physicians when necessary.
