Job summary

Healthforce in Colorado seeks a Licensed Practical Nurse for a travel assignment in Wray. The role is night shift 11:00 PM–7:00 AM at a skilled nursing facility, with guaranteed 40 hours weekly for several weeks starting 12/07/2026.

Responsibilities include administering medications, monitoring patients, assisting with daily living activities, and maintaining accurate records while coordinating with the care team. Housing and travel support available.

Qualifications

Active LPN license in Colorado.
CPR/BLS certification.
Prior experience in skilled nursing or long-term care preferred.
Ability to work night shifts and adapt to a fast-paced SNF environment.
Strong communication, documentation, and teamwork skills.
Proficiency with basic computer charting and electronic health records.

Responsibilities

Administer medications and treatments per physician orders and facility policy
Monitor patient conditions, document vitals, and report changes to the supervising RN
Assist with activities of daily living and support resident safety and comfort
Perform basic nursing procedures and delegated tasks under supervision
Maintain accurate medical records and charting in the designated system
Collaborate with the interdisciplinary team to implement and update care plans
