• Valid driver's license required for consideration

Responsibilities

• Post-acute long-term care/skilled nursing facility setting
• Patient ratio of approximately 1:17 with a census of 34 out of 61 beds
• Common diagnoses include CHF, dementia, ostomy, catheter care, depression, anxiety, diabetes, hypothyroidism, hyperlipidemia, hypertension, hemiparesis post-CVA, Alzheimer's, pain management, constipation, muscle weakness, A-fib, GERD, and edema
• Uses Point Click Care (PCC) EMR system
• 12-hour day shifts from 6a-6p
• Both 8-hour and 12-hour shifts available
• Every other weekend and every other holiday rotation required
• Location-specific orientation covers North and South units
• No on-call or call-back requirements
• Provide direct patient care including medication administration, wound and ostomy care, catheter management, and monitoring of chronic conditions
• Support patients with cognitive and physical needs including dementia care and mobility assistance
• Local candidates accepted at a reduced rate
• Candidates residing within 80 miles of the facility are considered local
