Qualifications

• Active LPN license in Maine (in good standing).
• Completion of an approved Licensed Practical Nurse program.
• Prior experience in long-term care or skilled nursing settings preferred.
• Ability to work onsite in Westbrook, ME during scheduled shifts.
• Strong communication, documentation, and teamwork skills.

Responsibilities

• Provide direct patient care to residents within the scope of an LPN license under supervision.
• Administer prescribed medications and document administration; monitor for adverse reactions.
• Monitor vital signs and patient status; report changes promptly to the supervising clinician.
• Assist residents with activities of daily living (ADLs) such as bathing, grooming, feeding, and mobility.
• Support wound care, catheterizations, and other routine clinical tasks as directed.
• Collaborate with the interdisciplinary team to implement and update care plans; chart accurately and ensure regulatory compliance.
