Job summary

Sanford Health is seeking a Licensed Practical Nurse to provide professional nursing care in long-term care facilities. The role requires travel within a 75-mile radius of the permanent address, with a minimum of 30 hours per month and at least one weekend shift per month.

The position offers a premium pay structure including $10/hour premium on all hours and shift differentials. 3 months of long-term care experience is required (6 months preferred).

Qualifications

• Graduate from an accredited practical nursing program and hold an unencumbered LPN license.
• Associate degree in practical nursing is preferred.
• Minimum of 18 years of age; must meet CEU and competency requirements.
• 3 months (6 months preferred) experience in Long Term Care is required.

Responsibilities

• Provides professional nursing care for residents under supervision of a RN/physician.
• Administers medications or treatments as ordered and documents outcomes.
• Performs care planning, assessments, and monitors resident status.
• Maintains confidentiality and communicates clearly with residents, families and health care team.
