Responsibilities

• Provide direct nursing care to residents in a long-term care setting, ensuring comfort and safety.


• Administer prescribed medications and perform basic treatments according to physician orders and facility procedures.


• Monitor residents’ conditions, document changes, and communicate with the supervising RN and interdisciplinary team.


• Assist with activities of daily living, mobility, and safety measures to support resident independence and well‑being.


• Maintain accurate charting, follow care plans, and participate in regular care plan reviews.


• Adhere to infection control practices, safety standards, and regulatory requirements; protect resident confidentiality.

Requirements

• Active LPN license in New Hampshire or eligibility to practice in NH


• Prior experience in a long-term care or skilled nursing setting preferred


• Ability to work the night shift schedule (18:45–07:15) and provide consistent, high-quality care


• Local resident or local commuting ability to the Franklin area (LOCAL ONLY)
