• Maintain charts and documentation in the electronic health record; ensure infection control.
• Respond to changes in condition, provide triage, and assist with emergencies as trained.

Responsibilities

• Deliver direct nursing care to residents in a long-term care/assisted living setting, following individualized care plans
• Administer medications and treatments per physician orders; monitor for side effects and document responses
• Conduct routine assessments, monitor vital signs, and report changes to the care team
• Collaborate with an interdisciplinary team to coordinate care, support activities of daily living, and ensure resident safety
• Maintain accurate charts and documentation in the electronic health record; adhere to regulatory requirements and infection control protocols
• Respond promptly to changes in condition, provide triage when needed, and assist with emergency procedures as trained

Requirements

• Active LPN license (in good standing); state licensure requirements for practice apply
• Prior nursing experience in long-term care, assisted living, or a similar setting preferred
