Qualifications

• Active LPN license in good standing; state licensure requirements apply.
• Prior nursing experience in long-term care or assisted living preferred.
• Current CPR/BLS certification.
• Ability to work in long-term care/assisted living and adapt to flexible scheduling.
• Comfortable with charting, medications administration, and multidisciplinary teamwork.
• Pass standard health screenings and background checks as part of the assignment.

Responsibilities

• Deliver direct nursing care to residents in LTC/assisted living per care plans.
• Administer medications and treatments; monitor side effects and document responses.
• Conduct routine assessments; monitor vital signs and report changes.
• Collaborate with interdisciplinary team to coordinate care and support ADLs.
• Maintain charts and documentation in the electronic health record; ensure infection control.
• Respond to changes in condition, provide triage, and assist with emergencies as trained.
