Job summary

Qualifications

• Current nursing license in the practicing state.
• Two years experience as an LPN/LVN in a clinical setting.
• Valid CPR certification.

Responsibilities

• Provide skilled nursing care to patients in their homes, under the direction of an RN.
• Implement individualized care plans in collaboration with patient and healthcare team.
• Educate patients and families on disease management and self-care techniques.
• Maintain accurate records of patient care and outcomes.
• Communicate with patients, physicians, and the care team regarding needs and status.
