Job summary

Redcrest Hospice in Indianapolis and surrounding areas is seeking a Registered Nurse (RN) for PRN hospice care. You will oversee patient care from admission to end of life, monitor needs, and work with an interdisciplinary team to maintain comfort and dignity for patients in their chosen environment.

You will perform autonomous nursing duties, document activities, and ensure adherence to state regulations and agency policies while supporting families and patients through the hospice journey.

Qualifications

  • Registered Nurse with current Indiana license.
  • Minimum one-year RN experience in acute care preferred.
  • Hospice experience is desired.
  • Valid driver's license and auto insurance in your name.
  • Capable of meeting physical demands.

Responsibilities

  • Oversee overall care of the patient from start to end of life.
  • Monitor patient condition and adjust care within scope of practice.
  • Work autonomously and follow physician-directive practices.
  • Coordinate care across interdisciplinary team and families.
  • Document all clinical activities per agency policy and Medicare guidelines.
  • Supervise aides and LPNs; participate in staff trainings.

As a Registered Nurse (RN), Hospice the work you do every day makes a difference in the lives of our patients. As a RN you will have the opportunity to provide one on one patient care and work at the top of your license. You will utilize your clinical skills in providing end of life care as directed by an attending physician. You'll be part of an interdisciplinary team that focuses on providing compassionate quality care to our patients.

HOW YOU'LL WORK:

You’ll oversee the overall care of the patient from beginning to end of life. You will observe and monitor the patient to determine if the level of care being provided meets the patients’ needs. You will work autonomously and be responsible for all practices and duties within the scope of practice outlined by the state.

MAJOR AREAS OF RESPONSIBILITY:
  • Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and make adjustments as needs change.
  • Minimize Patient Risk: Update all doctor orders in patient chart and administer medication as prescribed by the physician.
  • Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
  • Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members.
  • Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs. Attend in-service training and mandatory agency meetings.
HARD & SOFT SKILLS:
  • Compassionate communicator with a positive attitude.
  • Patience is a virtue when working with patients, families, physicians, and coworkers.
  • Attention to detail is critical, as is being observant and following directions.
REQUIREMENTS:
  • Registered Nurse with current license in the state of employment.
  • Minimum one-year experience as an RN in an acute care setting is preferred.
  • Hospice experience is desired.
  • Valid driver's license and auto insurance in your name as a driver
  • Capable of all physical demands
