Job summary

Healthforce is seeking a Social Worker – Medical Social Worker (MSW) for a traveling assignment in Helena, MT, covering hospital-based hospice care. This 13-week contract includes guaranteed 40 hours per week with 8-hour day shifts. Start date is 11/15/2026; travel to patient locations within the assigned radius is required.

The role requires an MSW (or in progress toward LCSW), active BLS, and at least 2 years of clinical/ED/MSW experience, plus a valid driver’s license for travel.

Qualifications

• Active driver’s license required.
• Active Basic Life Support (BLS) certification required.
• Master’s degree in Social Work (MSW) or in progress toward Licensed Clinical Social Worker (LCSW).
• Minimum 2 years of clinical/ED/Medical Social Worker experience.
• Ability to work in a hospice setting with adult patients; majority are 65+.
• Ability to lift up to 50 lbs intermittently.

Responsibilities

• Perform biopsychosocial assessments for hospice patients and families; contribute to care planning.
• Coordinate with interdisciplinary team to develop and implement care plans; support discharge planning.
• Provide psychosocial support, counseling, and education about goals of care, coping, and directives.
• Document assessments, plans, interventions, and progress in Epic EMR with HIPAA compliance.
• Assist with safety, mobility, and functional support; lift up to 50 lbs intermittently.
