The Pennant Group, Inc. in Rock Springs, WY seeks a Registered Nurse to join our hospice team. This position offers full-time and PRN/on-call options, with one weeknight per week and one weekend per month of on-call availability.

Qualifications

• Graduate of NLN-accredited school of nursing.
• Current licensure to practice as an RN.
• Strong observation, problem solving and communication skills.
• Ability to organize workload and work independently.

Responsabilités

• Performs comprehensive and ongoing patient assessments.
• Plans and directs patient care in home and community settings.
• Coordinates care with physician, family and interdisciplinary team.
• Provides patient and family education on self-care and treatment plans.
