Role

Travel Physical Therapist in a home health setting, responsible for patient evaluations and treatments in patients' homes and care facilities.

Responsibilities

Manage 3-5 patients daily with varied diagnoses including ortho, cardio, pulmonary, oncology, neuro, and geriatrics.

Qualifications & Skills

Minimum of 2 years healthcare experience, EMR proficiency with HomeCare Home Base (HCHB), and OASIS documentation expertise.

Key Duties

Conduct assessments, develop treatment plans, and document patient progress. Provide quality care while adhering to protocols.
