Job summary

Affinity Home Health Care Inc is seeking a Home Health Physical Therapist to evaluate and treat patients requiring rehabilitation services in a home health setting. You will develop individualized care plans, deliver skilled PT interventions, and monitor patient progress in coordination with physicians and the care team.

The role requires a Doctorate or Bachelor’s degree in Physical Therapy with NPTE pass, licensure in progress or active, and at least one year of PT experience.

Qualifications

• Licensed or actively pursuing licensure in the state.
• Minimum of one year of experience in physical therapy.
• CPR certification and a valid driver’s license with reliable transportation.
