Qualifications

Responsibilities

The Physical Therapist / PT is responsible for the assessment of referred patients requiring rehabilitation services, including:

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establishing and administering a treatment program with specific goals according to the patient's capacity and tolerance under the direction of the physician.
• Setting realistic and achievable goals for patients, documenting and recording the patient's condition, and educating patients and families in appropriate physical therapy methods.
• Developing a plan of care for each patient.
• Providing skilled physical therapy services/interventions per physician orders.
• Assessing patient needs and evaluating and modifying care to meet therapy goals.
• Collaborating with all disciplines to plan and evaluate team goals for each patient.
