Job summary

Elara Caring seeks an Occupational Therapist to join our Houston home care team. You will evaluate patients, create and implement Plans of Care, and coordinate with a multidisciplinary team to ensure quality care and timely discharge planning.

Travel up to 50% within the branch coverage area; must have a reliable vehicle and valid license. Base salary $95,000–$117,000 with productivity bonus opportunities and comprehensive benefits.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one year of experience in a clinical care setting.
• Willing to travel within the branch coverage area (50%).
• Must have a dependable vehicle and valid driver’s license.
• Auto insurance in accordance with state laws.

Responsibilities

• Evaluates the OT patient and develops a plan of care.
• Implements and develops the plan of care for quality and discharge planning.
• Performs initial and ongoing therapy evaluations and re-evaluations.
• Conducts follow-up assessments according to regulatory guidelines.
• Communicates findings to supervisor and care team.
• Instructs patients and families in activities of daily living and safe home programs.
• Supervises COTA/HHA and clinical staff to ensure care plan adherence.
• Maintains documentation per standards; assists with equipment needs.
