Job summary

Elara Caring is seeking an Occupational Therapist to provide home-based therapy with a focus on improving daily living activities and function. You will assess patients, develop and adjust plans of care, and collaborate with a broad health care team to deliver high-quality home health services.

The role requires traveling within the branch area, lifting up to 50 pounds, and adherence to physician orders and care standards.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one year of experience in a clinical care setting.
• Able and willing to travel 50%.
• Must have a dependable vehicle and a valid driver’s license.

Responsibilities

• Evaluates the occupational therapy patient to determine rehabilitation needs and develops a plan of care.
• Implements and updates the Plan of Care to ensure quality and continuity of care and proper discharge planning.
• Communicates significant findings to the supervisor, physician, and care team.
• Initiates patient safety plans using patient, family, and community resources.
• Performs occupational therapy services per standards and physician orders.
• Instructs patients and families in activities of daily living and uses adaptive equipment.
