• Ascension Criminal Background Check Results, Education Verification, and Employment Verification required for onboarding.
• Health histories and personal documentation required for onboarding: MMR, Hepatitis B, Influenza Vaccine, Varicella, Tdap, 2 Step PPD or IGRA (Quantiferon/Tspot), Fit Test, and Health Statement.
