Qualifications

• 1 year of experience in a skilled nursing facility or geriatric rehabilitation; travel experience considered

Responsibilities

• Deliver occupational therapy services to residents under physician orders and the established therapy plan
• Assess patient needs, develop treatment plans, implement interventions, and re-evaluate progress toward functional goals
• Document progress notes, treatment plans, and discharge planning in accordance with facility and regulatory requirements
• Collaborate with an interdisciplinary team including OT, PT, nursing, and social work to support patient outcomes
• Ensure safe, compliant practice with infection control, equipment maintenance, and privacy standards
• Provide education to patients, families, and staff to promote independence and safe activity performance
