Job summary

Healthforce Healthcare LLC is seeking a Certified Occupational Therapy Assistant for a travel assignment in a skilled nursing facility in San Leandro, CA. The contract runs for 13 weeks with guaranteed hours and a 5x8 day shift (08:00–16:30).

Start date is 01/03/2027 with weekly pay between 1,618 and 1,659 and 35 guaranteed hours per week. The role requires an active CA COTA license or eligibility, NBCOT certification, and an ACOTE-accredited degree, plus 1 year of SNF or geriatric rehab

Qualifications

• Active California Certified Occupational Therapy Assistant license or eligible for licensure
• NBCOT certification as a COTA
• Associate degree in Occupational Therapy Assistant from an ACOTE-accredited program
• 1 year of experience in a skilled nursing facility or geriatric rehabilitation; travel experience considered

Responsibilities

• Deliver occupational therapy services to residents under physician orders and the established therapy plan
• Assess patient needs, develop treatment plans, implement interventions, and re-evaluate progress toward functional goals
• Document progress notes, treatment plans, and discharge planning in accordance with facility and regulatory requirements
• Collaborate with an interdisciplinary team including OT, PT, nursing, and social work to support patient outcomes
• Ensure safe, compliant practice with infection control, equipment maintenance, and privacy standards
• Provide education to patients, families, and staff to promote independence and safe activity performance
