Qualifications

• Active Missouri OT license and at least 1 year of OT experience.
• OT-MO license (Copy needed).
• OTR-NBCOT certification; BLS-AHA certification (Copy Needed).
• SSN required; DOB required.
• Work history: no more than 90 days gap in last year.

Responsibilities

• Perform occupational therapy evaluations.
• Develop personalized treatment plans.
• Assist with activities of daily living.
• Document progress using Meditech EMR.
