Responsibilities

• Assess, plan, implement and coordinate treatment processes from diagnosis through follow up.
• Guide patients through collaboration with physicians and care teams.
• Perform chart reviews and data analysis for performance improvement.
• Develop and implement evidence-based policies for the program.
• Provide education to patients and family members during hospitalization.
• Participate in patient care rounds and ensure regulatory compliance.
