Intermountain Health is seeking a Clinical Program Coordinator in the Trauma department at Good Samaritan Hospital (Lafayette, CO). The role oversees program operations, ensures alignment with evidence-based guidelines, and educates patients and families throughout the hospital stay and post-discharge follow-up.

Requires a Bachelor's in Nursing, active RN license, and 3 years of RN experience; BLS-HCP certification is required upon hire or within 60 days.

Qualifications

• Bachelor's degree in Nursing required
• Current RN license in state of practice
• BLS-HCP certification upon hire or within 60 days with approval
• Three years relevant RN experience

Responsibilities

• Coordinate treatment processes from diagnosis through follow-up with physicians, nurses and support programs.
• Educate patients and families during hospital stay and serve as a resource after discharge.
• Actively participate in patient care rounds and ensure care meets evidence-based guidelines.
• Review data and perform analyses for performance improvement activities in the program.
• Develop and implement evidence-based policies and standards for the program.
• Maintain data and act as subject matter expert; create a QA program.
• Promote Intermountain Health's mission and values; may float to other departments as needed.
• May be on-call during regularly scheduled shifts.
