Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification is required.
• Banner requires a minimum of one year of acute care hospital experience within the past 12 months.

Responsabilités

• Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulate a plan of care including discharge planning with input from patient/family and health team.
• Implement the plan through direct patient care, coordination, and supervision of the care team.
• Evaluate patient and family responses to care and revise the plan as needed.
• Document assessment, planning, implementation and evaluation in the patient record.
