Job summary

Catholic Charities Diocese of Rochester (CCDOR) is hiring a full-time Service Coordinator to support individuals in the TBI and NHTD Waivers. The role emphasizes person-centered service planning, documentation, and collaboration with program staff across regions in Monroe and surrounding counties.

You will engage in outreach, client advocacy, and coordination of public benefits, ensuring trauma-informed care and adherence to DOH guidelines.

Qualifications

• Bachelor’s degree and one year of experience providing case management/service coordination and information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors.
• OR
• Associate’s degree and two years of experience providing case management/service coordination, information, linkages and referrals regarding community-based services for individuals with disabilities and/or seniors.
• Paid employment and unpaid experience (volunteer work, internships) can be used to fulfill requirements for Service Coordinator.

Responsibilities

• Provide service coordination to individuals within the TBI Waiver and NHTD Waiver programs living in Monroe, Finger Lakes or Southern Tier Regional Counties.
• Maintain accurate and verifiable documentation of all services rendered.
• Develop and complete comprehensive service plans and DOH-required documentation using a person-centered approach.
• Demonstrate cultural competency and support diversity.
• Provide monthly face-to-face contacts with participants, including home visits quarterly.
