Job summary

Catholic Charities Family and Community Services is hiring for a Full-Time role focused on intake and ongoing care management for individuals with chronic illnesses in the Monroe, Finger Lakes or Southern Tier regions. The role requires collaboration with program staff to ensure trauma-informed care within a compassionate, client-centered framework.

Qualifications

• Bilingual (English/Spanish) is required for some positions.
• Two to four years of practical experience with individuals with disabilities or chronic illnesses.
• A combination of education and experience may be substituted at the director’s discretion.

Responsibilities

• Provide intake and ongoing care management services to individuals with one or more chronic illnesses.
• Maintain accurate documentation of all services rendered.
• Complete comprehensive assessments and care plans per DOH standards.
• Coordinate with Health Home staff to ensure trauma-informed care.
• Provide client advocacy and connect to public benefits and community resources.
