Intermountain Health in Denver, CO seeks a School Social Worker to conduct psychosocial assessments, develop treatment plans, and support IEPs for day treatment patients under LCSW supervision.

You will collaborate with families, healthcare teams, insurers, and community resources to coordinate care, advocate for students, and ensure timely documentation and discharge planning. A MSW and CO licensure are required.

Qualifications

• Must hold a Master of Social Work degree from an accredited program.
• Current LSW licensure or equivalent is required.
• Colorado SSP/TEE licensure eligibility within 30 days of employment.
• Strong written and verbal communication skills; proficient with computers.

Responsabilités

• Supervise by an identified LCSW; participate in supervision meetings and chart reviews.
• Perform psychosocial assessments and provide psychotherapeutic interventions.
• Screen behavioral health symptoms with assessment tools; maintain documentation.
• Coordinate discharge planning and collaboration with internal/external services.
• Ensure timely, accurate documentation in the medical record and meet productivity standards.
