Job summary

Intermountain Health in Denver, CO, is seeking a School Social Worker for Mount St Vincent Home. The full-time role supports psychosocial assessments, IEP collaboration, discharge planning, and care coordination.

You will work under supervision of an LCSW and provide trauma-informed psychotherapeutic interventions to day treatment patients. The position requires an MSW, current Colorado social work licensure, SSP/TEE authorization within 30 days, and BLS/First Aid within 90 days.

Qualifications

• MSW degree from an accredited program.
• Current State Licensure as a Licensed Social Worker or equivalent.
• Current Colorado CDE licensure as SSP or TEE Authorization within 30 days of employment.
• Excellent written and verbal communication skills.
• Proficiency in computer applications.
• Positive driving record with valid Colorado driver’s license and auto insurance.
• Basic Life Support and First Aid within 90 days of employment.

Responsibilities

• Works under supervision of an identified LCSW with required supervision meetings and chart reviews.
• Uses trauma-informed care to build relationships and respond to patient needs.
• Conducts psychosocial assessments and provides psychotherapeutic interventions aligned with treatment plans.
• Administers assessment tools to screen behavioral health symptoms and diagnoses.
• Coordinates discharge planning with internal and external services for ongoing care.
• Documents accurately and timely in the medical record to support care communication.
• Meets productivity standards and expectations.
• Performs other duties as assigned.
