Nexus Family Healing is hiring a School Case Manager to support students and families in the Minot, ND area. The role focuses on coordinating access to medical, social, educational, and other services within a school setting.

The position emphasizes trauma-informed care and collaboration with school districts to promote resilience and safe outcomes for youth. Benefits and on-site work are provided in a 12-month placement.

Qualifications

• Bachelor's degree in Human Services required; Social Work preferred.
• 1–3 years of work experience serving children and families preferred.
• Current North Dakota social work licensure preferred; driver’s license required.

Responsibilities

• Coordinate all tasks to meet treatment needs and regulatory requirements.
• Complete systemic assessment of child and family needs.
• Document case management and treatment planning within timelines.
• Provide crisis intervention as needed to ensure safety and best interests of child and family.
• Promote trauma-informed, trauma-sensitive practices across the team.
