Job summary

The RN Case Manager: Victim Services at Community Health Center of Southeast Kansas, Inc. (CHC/SEK) is a key member of the Trauma Recovery Medical Home (TRMH) care team, delivering trauma‑informed nursing care and coordinating services for victims of IPV and sexual assault across seven rural counties in Southeast Kansas.

The RN conducts intakes, builds care plans, and ensures wraparound support for patients and their children.

Qualifications

• Current Kansas RN license required; must obtain OK license within one year of hire.
• Minimum three (3) years of nursing experience, preferably in emergency, women’s health, or similar role with advanced assessment skills.
• At least 1 year of case management, or experience relevant to community health, correctional health, or cross-agency collaboration.
• Must complete forensic nurse training (SANE and IPV) within 6 months of hire.
• Victim Services and Trauma-Informed Care training must be completed within 3 months of hire.
• Demonstrated leadership experience.
• Basic Life Support (BLS) certification through American Heart Association or obtain within 90 days of hire.
• Must possess a valid driver’s license as travel is required.

Responsibilities

• Provide trauma-informed nursing care and case-management for TRMH participants, including forensic exams.
• Co-facilitate survivor-focused life skills workshops and peer-support groups with the TRMH team.
• Represent CHC/SEK in Coordinated Community Response Team meetings and collaborate with community partners.
• Support data collection, grant reporting, HIPAA compliance, and ongoing program evaluation.
• Perform other duties as assigned by the program leadership.
