Job summary

CHC/SEK is seeking a dedicated RN Case Manager: Victim Services to support trauma-informed, survivor-centered care across a seven-county rural region in Southeast Kansas. The role combines clinic-based nursing, case management, and advocacy for victims of IPV and sexual assault.

The RN will work with medical and mental health teams, lead life-skills sessions, and coordinate with victim advocates and CRTs to ensure wraparound support for survivors and their children.

Qualifications

• Current Kansas RN license; must obtain Oklahoma license within 1 year.
• Minimum three years nursing experience, preferably emergency or women's health.
• At least 1 year case management or cross-agency collaboration experience preferred.
• Complete forensic nurse training (SANE/IPV) within 6 months.

Responsibilities

• Provide trauma-informed nursing care and case management for TRMH participants via phone, telehealth, and in-person visits.
• Coordinate medical, dental care, referrals, and follow-up for victims and their children.
• Develop individualized care plans and maintain regular follow-up to meet patient goals.
• Support data collection, reporting, and compliance with HIPAA and safety standards.
