Job summary

Delta Family Services seeks a dedicated professional to work with Medical Resource Parents to support well-being, stability and permanency of medical-level foster children in home, school, and community settings.

The role emphasizes trauma-informed coaching, collaborative partnering with RP and case managers, and thorough documentation to ensure individualized care plans are met across medical, behavioral and educational domains.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Sociology, Guidance and Counseling or Child Development with at least 1 year experience.
  • Experience working with families and children in community-based and home-based settings; knowledge of medical systems preferred.
  • Strong assessment and documentation skills with crisis-management capabilities.

Responsibilities

  • Provide direct support, consultation and coaching to Medical Resource Parents to address medical, emotional, behavioral, social, academic, developmental and physical needs.
  • Deliver trauma-informed, strengths-based, individualized parent training and coordinate health services for children.
  • Collaborate with Medical RP, CUA CM, medical providers and educational teams to ensure appropriate placements and services.
  • Support transportation planning and provide 24/7 on-call crisis management for resource parents.
  • Maintain medication logs and documentation; assist with Medical Treatment Plans and updates.
